Cart Transportation Service

Cart transportation service for academic-related reasons is available for students with permanent and temporary mobility limitations and who have appropriate documentation of disability. Cart transportation is not an on-call service, students must complete the process below to request cart services.

First Time Cart Users

If you are requesting cart transportation services, please read, complete, and submit the following required forms:  

Student

1. Application for Services (PDF)
2. Verification of Disability Form (PDF)
3. Cart Service Request Form for Students (Classes/Recurring)
4. or Cart Service Request Form for Students (Single-Event)

Faculty & Staff

1. Cart Service Request Form for Faculty, Staff, Alumni, or Special Guest

Note: Cart Service Request Form requires requesters to be logged into their SDSU account.

Continuing Cart Services

If you have used cart services in the past and need to reestablish or change your cart transportation services, please complete and submit the appropriate Cart Service Request form: 

• Cart Service Request Form for Students (Classes/Recurring)
• or Cart Service Request Form for Students (Single-Event)

For adjustments or corrections to previously scheduled cart service, please send an email to [email protected].
